4Square Counseling collects information regarding how the website is accessed. This usage data may include your computer’s Internet Protocol Address (Your IP address), your browser type and version, which pages of the website you access, how much time you spend on the pages of the website, the time and date you visited, what type of device you use to view the website, and other diagnostic data.


When visitors leave comments on the site I collect the data shown in the comments form, and also the visitor’s IP address and browser user agent string to help spam detection.

An anonymized string created from your email address (also called a hash) may be provided to the Gravatar service to see if you are using it. If you upload images to the website, you should avoid uploading images with embedded location data (EXIF GPS) included. Visitors to the website can download and extract any location data from images on the website

4Square Counseling uses Google Analytics, a service which transmits website traffic data to Google servers in the United States. Google Analytics does not identify individual users or associate your IP address with any other data held by Google. 4Square Counseling uses reports provided by Google Analytics to help me understand website traffic and web page usage.

You have the option to opt out of Google Analytics cookies altogether by downloading and installing the Google Analytics Opt-Out Add-on for your browser.

If I become aware that a Child has left personally identifiable information on the Website without parental consent, I will delete it from my servers.

How long we retain your data

If you leave a comment, the comment and its metadata are retained indefinitely. This is so I can recognize and approve any follow-up comments automatically instead of holding them in a moderation queue.

If you send an email, the email is retained indefinitely. This is so I can reference back to our conversation if needed. This information is printed and stored in your personal file folder in a locked file cabinet.

Booked appointment information, intake forms and agreements, action plans, and termination letters are retained indefinitely. Per HIPAA regulations, I am required to keep this information. This information is printed and stored in your personal file folder in a locked file cabinet.

What Rights You Have Over Your Data

 If you have left comments on this website, you can request to receive an exported file of the personal data we hold about you, including any data you have provided to us. You can also request that we erase any personal data we hold about you. This does not include any data we are obliged to keep for administrative, legal, or security purposes.

Where I Send Your Data

How I Protect Your Data

Your data, with the exception of comments left on blog posts is almost never stored on a computer, or on the net. Your identifying data is stored as a printed copy in your personal folder in a locked file cabinet. Your name and phone number is stored on my business phone during our contracted relationship. Your email address is stored in my email program during our contracted relationship.

What Data Breach Procedures We Have In Place

 In case of a data breach, break in of my office, or theft of my phone, you will be notified immediately through email, or phone call.

“Do Not Track”

I do not support Do Not Track (”DNT”). You can enable and disable “DNT” by visiting the preference or settings of your web browser.
